Both recipient and donor can then develop complete livers - because the liver can restore itself unlike other organs (excepting maybe skin). The transplant is not a cure - but the healthy liver takes a very long time to damage and with good care, that can be minimized.

While I disagree with the underlying assumption (which we can debate elsewhere), I do think this is a good way to look at it.

Jim had a bad day today... vital signs not as good as they were, but still within tolerance to keep him on the recipient list. If he continues to... if he doesn't turn things around, he'll be dropped from the list.
It's quite a balancing act, it seems... he has to be in critical condition in order to need the transplant, but he has to be healthy enough to tolerate the surgery, and he's "right on the edge".The first to state his case seems right until another comes and cross-examines him.
