I hate this phrase. As I see it, there are two options for response:
1. Be honest
2. Be dishonest
Nobody wants you to go the honest route--and admit it, you don't want anyone else to go the honest route when you ask them this, either.
"How are you doing today?"
"Oh man, I'm having a rough day today. Let me tell you about..."
"Whoa dude, I just asked how you were doing. No need to get all serious and stuff."
So instead we go the dishonest route:
"How are you doing today?"
"Oh, I'm great!"

I've experimented with an honest, yet still optimistic response: "I'm doing ok." But then people will say something like, "Just ok?" and the cycle begins anew.
More research is necessary.
