I'm figuring that either he forgot that the voice coming into his ear from that side telling him what to say and do, was someone actually standing beside him, or else his keepers promised he could have ice cream when he finished and he turned around expecting it immediately afterwards

After watching slightly different angles of this "phantom handshake" event, I've concluded Biden was gesturing to someone in the audience. He actually points a couple seconds later.

First, as he he starts to leave the podium, his eyes open wide first (as though seeing someone he recognizes) - and THEN he puts his hand out and appears to be looking a bit into the distance. Then when he is walking away, he similarly points to someone off camera.

An explanation even none his supporters have offered so far. It still does not account for his extending his hand to shake unless you're saying he thought that person off stage was next to him.

If anything, it looks like he expected someone to be there, was confused when they weren't and was told they were on the other side (why he turns and points that way like confirming that they're over on that side).

Mimic the facial expression and hand movement. See if it feels like the preliminary to a hand shake. It certainly doesn't to me.

You must be able to see far more of his face to read expressions than the quarter view afforded in the video.

Looks like the issue is that you watch too much if you think you can accurately read facial expressions from that angle.

It was an odd gesture, for sure. But everything about this hair-sniffer is odd.

Maybe. But the way his eyes opened wide first indicated to me he saw someone he recognized. But your explanation could be correct. Who knows. Surely not Biden. He probably doesn't even remember any of this happened by now.
