People like Tom, the guy with the same insurer who makes all the right life choices and is in perfect health and thus pays more into his insurance than he ever uses. He and others like him are ultimately providing the insurer with all the money they need to pay for Bob's treatment.
50% of all healthcare costs are incurred by people in the final 2 years of their lives. So the whole "makes all the right life choices" thing is an irrelevant talking point. Some people live shorter lives as a result of their habits. But everyone gets old eventually, even people who made the 'right choices'. And when they do, that's when it really costs the serious money.

Healthcare for all makes simple sense because we all get old and eventually need healthcare, and it makes sense to spread the financial risk over the whole population rather than trying to extract millions of dollars from a sick elderly person for their own treatment.
